This Telehealth Consent applies to services accessed through MedStation Wellness, operated by MedStation Card LLC (“MedStation,” “we,” “us,” or “our”).

By using this website and engaging with telehealth services facilitated through MedStation Wellness, you acknowledge and agree to the following:

  1. Nature of Telehealth Services

Telehealth services involve the delivery of healthcare services using electronic communications, including online questionnaires, secure messaging, audio, or video consultations.

MedStation Wellness does not provide medical care. Telehealth services are delivered exclusively by independent licensed healthcare providers through a third-party telehealth platform operated by MD Integrations Medical Group PC.

  1. Provider Relationship

All medical services, evaluations, diagnoses, treatment decisions, and prescriptions are performed solely by independent licensed healthcare providers.

These providers practice medicine independently and are solely responsible for the medical care they provide. MedStation Wellness does not control or interfere with clinical decision-making.

  1. Benefits and Risks of Telehealth

Potential benefits of telehealth may include increased access to care, convenience, and efficiency.

Potential risks may include, but are not limited to:

  • Technical failures or interruptions

  • Incomplete or inaccurate information provided by the patient

  • Limitations in diagnosis or treatment compared to in-person care

You acknowledge and accept these risks when choosing to participate in telehealth services.

  1. Alternatives to Telehealth

You understand that you may choose to seek care through traditional, in-person healthcare services instead of telehealth at any time.

Participation in telehealth services is voluntary.

  1. No Guarantee of Treatment or Prescription

Engaging in telehealth services does not guarantee that a healthcare provider will determine you are eligible for treatment or prescribe any medication.

Prescriptions, if any, are issued solely at the discretion of the licensed healthcare provider based on medical appropriateness.

  1. Emergency Situations

Telehealth services are not appropriate for medical emergencies.

If you experience a medical emergency, you should call 911 or seek immediate emergency medical care.

By using this website and proceeding with telehealth services, you acknowledge that you have read and understood this Telehealth Consent and voluntarily consent to participate in telehealth services as described above.
